FAQ's

 

Can I see the doctor the same day if I have a problem?  On most days, yes.  We allow for walk-in time slots every day we are in the office.  Please call ahead.  Make an appointment here.

 

How do I have my medical records transferred?  Click here to print out our Release of Medical Information form.  Complete the form and present it to the health care institution or provider you want to transfer records to/from. It’s often effective to pick up a copy in person and bring them to your office visit — that way you have your own copy.  Be aware that some providers or health care institutions may charge a record copy fee.

Does the doctor take my insurance?  We take most insurances.  Please call or go online to your insurance company’s website and see if we are listed as a provider. 

Does the doctor see children?  We see age 18 and above.  

What about prescription refills?  Please ask your pharmacy to submit refill requests electronically (e-scribe) or by fax at 708-589-9059.  Refills requested after hours are done only in cases of urgent need and at the discretion of the healthcare provider.  Please also refer to our Prescription Policies.
