Patient Portal --

an online tool for your health

A patient portal is a website for your personal health care. It helps you keep track of your health care provider visits, test results, billing, prescriptions, and so on. You can also e-mail your provider questions through the portal.

 

For access, you will need to set up an account. The service is free. A password is used so that all of your information is private and secure.

Portal E-mail: The patient portal is secure and encrypted.  Please e-mail us with any questions through the patient portalAs e-mails are only checked on a periodic basis throughout the day or the next day, we require that e-mails are restricted  to non-emergent questions only.

Refills: You can e-mail us through the portal with refill requests.  Sometimes it's easier to call your pharmacy and ask them to generate an e-mail or fax request and be sure that they have our new fax number (708) 589-9059.  If its an urgent need for a refill, please call the main office number at (708) 603-5980.

Insurance and Billing Questions: Online bill payment is available through the patient portal.  You can also e-mail us with questions.  Otherwise, please call the general office number at (708) 603-5980, and select option 3 for billing and insurance. 

To Reach Our Clinicians: Please note, the online portal is NOT for medical emergencies.  For non-urgent questions during regular business hours and after hours please call the above office number, (708) 603-5980.  After hours the general office number will forward to our Answering System.  Please follow the instructions with your phone keypad, and it will allow you to create a message that will forward to your provider. 

For Emergencies: If you are in need of emergency care, call 911 immediately or go to the nearest emergency room.  Please also call the office and notify us of any urgent or emergent issues.
